Frailty and Falls in the Community
As reported in the iHub Connect Newsletter, working in partnership with Argyll & Bute, Fife, Glasgow City and West Dunbartonshire, the ihub's Living Well in Communities team are currently testing community based interventions that enable people with frailty to live in the community.
Improving the identification of people living in the community at risk of frailty or falls is a constant on the health and social care agenda. This ihub project aims to support Health and Social Care Partnerships test and evaluate new pathways of care that enable people at risk of frailty or falls to live healthier lives in the community that would otherwise have been spent in hospital.
