The 'Home NOT Hospital' practice exchange event provides an opportunity to discuss how housing is helping make better use of hospitals.
It will:
- Showcase partnership approaches to avoiding admission, and faster discharge from hospital
- Build momentum through sharing examples of effective practice
- Share ideas and drive local improvement.
